"Full-Sized Futon" to me means a futon mattress which is too thick for a mortal to "roll-up," and is 54" wide x 75" long. Also known as a "double," and in contrast to a "twin," "queen," or "king."

I would say that, in Japan, a futon is something like choice #2. In Pittsburgh, though, I think "futon" generally translates into "Any form of mattress with a frame that can be converted between couch-form and bed-form." And as for "full-size", well, I concur with leko.
